In order to ensure the continuous wellbeing of the VHPS Community, as per the current prevention and control measures in UAE (declared as Public Health Emergency of International Concern), we require you to provide information regarding your child's history of (or planned) travel during (or immediately prior to) the scheduled half-term break (October 14th - 23rd 2020).

Note: The form is mandatory for each student and has to be filled individually irrespective of siblings.
